We know that privacy is important to you, and our goal is to maintain your trust and confidence when handling Personal Information (PI) and Personal Health Information (PHI) about you. The request to collect is voluntary; however, can be mandatory under applicable laws in order for Free Agents NH, LLC to assist you and/or enroll you.

Information Collected and Stored Automatically:  When you visit our website, and use any links of external website pages, we will gather and store certain information about your visit automatically. Some of the PI we collect includes your legal name, phone number, Internet address and geographic location. This information is used only to help us follow up with those that visited our website. Legal Authority to Collect:  As licensed agents/brokers through the NH Department of Insurance, certified with the Federally-facilitated Marketplace and the agency is certified with all insurance companies that we offer; we are legally authorized to collect and store PI and PHI from Free Agents NH, LLC clients or prospective clients.

Authorized Users and Disclosures:  To protect you against unauthorized access to PI and/or PHI, Free Agents NH, LLC has adopted stringent security standards with multiple layers of security controls and monitoring. To protect the privacy, confidentiality and security of your data, we store all PI and/or PHI in a safe area, with limited access to anyone within the company and unauthorized access from outside of the company. The only authorized users are Free Agents NH, LLC’s President and licensed agent/s.

Free Agents NH, LLC treats all PI and PHI data as confidential and will disclose such information only according to our strict privacy and confidentiality policies and federal and state laws designed to maintain privacy, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy Rule. We will not disclose to third parties any identifying data, including but not limited to name, mailing address, e-mail address and phone number, unless you authorize such disclosure or such disclosure is permitted or required by law.

Rights to Accessing and Making Changes:  All clients of Free Agent NH, LLC have the right to access and/or make changes to the PI and/or PHI information that Free Agents NH, LLC has stored, at any time. Upon request, corrections and/or amendments must be completed within 10 working days. Request for access (copies) of the PI and/or PHI information stored must be determined acceptable or denied and then responded to in writing within 30 days. The method of supplying this information to you must be specific in the request. PI & PHI will NOT be sent via email.
