In order to keep you informed regarding ATM activities, we need to maintain current contact information for ATM members. Please use this form to notify the ATM office of any changes or additions to your contact information. For associate members, submitting this form will not update the information published for you as a part of the Find A Midwife function of the ATM website.
If you would like to update or add more than one type of email, address, or phone, you can utilize the comments section, or submit this form multiple times.

Prorated Rates
Memberships shall be renewed each January. If you are renewing in the middle of the year you may prorate your membership dues.

The prorated amounts will be reflected on the payment form. If mailing your application, contact the ATM office if you have any questions regarding the cost of your membership.
